Everyone around you seems so confident and prepared while you are a bundle of nerves. You’ll begin questioning whether you belong here. Newsflash—you do. The truth is everyone has doubts.

Here’s what the brochures didn’t tell you about that first month: the exhaustion, the comparison spiral, the logistics nobody explains, and how to manage the transition. Consider this a debrief from everyone who’s already been where you’re standing.

The First-Month Overwhelm Hits Different

Week one doesn’t ease you in. It feels more like the floodgates have opened. The first shock is the sheer volume of material. It’s lecture on lecture, lab on lab, with little time in between to let things settle.

Friendships move fast too. The person you thought you clicked with at orientation already has a study group while you’re still learning names. It’s not personal. Remember, everyone is struggling to find their footing.

Free time feels like it has evaporated into thin air. Evenings become “one more read-through” while weekends feel less like rest and more like triage. What no one says out loud is that everyone feels overwhelmed; even the confident students are secretly googling the same thing as you, just maybe more quietly.

Friendships and Relationships Under Pressure

Funny things happen to friendships when you put a hundred stressed, high-achievers in a room together. Study groups can quickly turn into scoreboards. There’s always the person who has finished the reading, color-coded their notes, and is already three steps ahead. It’s hard not to fall into the trap of measuring yourself against others.

Often it’s your older friendships that take the first hit as plans get canceled and texts sit unanswered. The friends worth keeping are the ones who don’t need explanations. They understand and accept “I’m alive, just drowning a bit” as a legitimate update.

Not every relationship needs to be deep and meaningful. Some connections just need to be low-effort and steady. Hold onto those.—they matter. Let the rest slide for now. Everyone around you is carrying more than they’re showing. You’re not the exception.

The Practical Logistics Nobody Mentions

Nobody tells you finding a study spot is its own competitive sport. Get to the library after 10 am and it’s game over—every desk and outlet gone. Try to scope out backups early, whether it’s the dead floor no one uses, a forgotten side room, or the hospital cafe during the quiet hours. Having a fallback means that you aren’t left stressed when you arrive late.

Food is another potential battleground. Preparing nutritious meals is good in theory until the alarm goes off at 6:30 am and you’re faced with a fridge full of ambition you never quite managed to execute. Focus on things like eggs, frozen vegetables, fruit, and yogurt—anything that takes under ten minutes. It may be boring and repetitive, but it’s sustainable.

There are resources on campus to help you through this period, but often aren’t well advertised. Find out about learning support, counselling, peer mentoring, and quiet rooms in week one before you need them and while you still have some spare time. Your future self will thank you.

The “Everyone Else Is Smarter” Spiral

Almost everyone feels this at some stage. As you sit watching a classmate rattle off an answer without even pausing for breath, your brain decides that your admission was the result of a paperwork mix-up or admission error. What you don’t realize is that classmate is thinking the same thing about themselves.

Feeling like an imposter is not unusual during those first few weeks of medical or nursing school. It’s just what happens when you put a group of high achievers in the same room learning a huge volume of complex material. The most useful way to combat it is to stop comparing yourself to others and focus on your own trajectory.

Find a second- or third-year student and ask them, honestly, if they felt this too. Nearly all of them did. Start logging tiny wins, like grasping something you’ve been struggling with or nailing a lab. They’re proof you belong on the days your brain won’t listen to reason.

Early Warning Signs and What to Do First

Burnout and isolation rarely come with sirens. The signs are much quieter and more insidious: exhaustion sleep doesn’t touch, irritability that feels out of character, pulling away from people, or the sense that every ordinary task has become enormous.

First responses should be simple. Tell someone your day was rough. Step outside for ten minutes of fresh air and daylight. Eat something that didn’t come out of a machine. Book the counselling service earlier than feels necessary.

It’s important to avoid becoming isolated. Just sitting in a common area around other people can work wonders. Small connections interrupt the spiral more effectively than waiting until everything feels impossible.

Keeping Perspective

There will be days when you seriously question whether this was the right decision. That’s part of the process. It’s not proof you got it wrong. The students who last aren’t the ones who never struggle. They’re the ones who keep showing up while the struggle is still happening.

The first month is tough and often exhausting, but it doesn’t last forever. You’ve already done the hardest part by starting.

<details class="wp-block-details is-layout-flow wp-block-details-is-layout-flow">
References

https://www.osmosis.org/blog/overcoming-impostor-syndrome-in-medical-school

https://evolve.elsevier.com/studentlife/peer-support.html

https://studenthub.elsevier.com/en/wp-content/uploads/sites/10/2025/12/BTU-Medical_Student_Guide_Final_Dec2025.pdf

https://www.osmosis.org/blog/2023/10/30/how-to-build-study-habits-that-last

https://www.osmosis.org/blog/2021/08/30/the-osmosis-ultimate-guide-to-studying-styles

Burnout in Undergraduate Nursing Students – Clinician Well-Being Knowledge Hub

https://samajournals.co.za/index.php/ajhpe/article/view/1559

A support framework to facilitate social learning and integration skills of first-year undergraduate medical students | African Journal of Health Professions Education

Imposter syndrome: preventing it, overcoming it and the link to burnout – The DO

https://www.ama-assn.org/medical-students/succeed-medical-school/vital-tips-help-manage-medical-school-s-stressful-first

https://students-residents.aamc.org/medical-student-well-being/navigating-imposter-syndrome

What I Wish I Knew Before Starting Med School Year One

MS1 Month-by-Month Mental Health Checkpoints and Course C…

9 Actionable Tips for Nursing Students in Your First Year | Alliant University

https://www.registerednursing.org/articles/nursing-school-stress

</details>

By: Lynda Malcolm, MD, JD Somewhere in your first few weeks, there’ll be a moment when you start to doubt yourself.
